Insurance Coverage

We must obtain a copy of your current valid insurance to provide proof of insurance. If you have insurance coverage for our services, we will work with you to help you understand your insurance benefits and your personal financial obligations regarding your services at our office. As a courtesy to you, we will verify your insurance coverage and, in most cases, file your insurance claim for you;  however, we must emphasize that as medical providers, our relationship is with you, not your insurance company. You are ultimately responsible for payment of your services in our office.

Steps to Services

  • Complete Intake and Consent Forms provided to you by our intake coordinator.

  • Submit Physician's Referral:

    • A referral from your physician is required before we can begin therapy.  Please ask your physician to fax a referral to our fax 407-386-8466.  If you have a copy, you can take a picture and email it to

  • Are you transferring services?

    • If your child has received therapy from another agency within the last 6 months, a copy of the evaluation and care plan from the previous agency is also required before we can complete a benefit check.  Please email copies to

  • Benefit Check:

    • If you are utilizing your commercial insurance carrier, we will check your insurance benefits and request authorization (if applicable) from your insurance company. Please note, this can be a lengthy process and can unfortunately take from days to weeks depending on your insurance provider.

  • Scheduling the Evaluation:

    • Once all the required documentation has been completed, we will contact you directly to schedule an evaluation.

What is an evaluation?

After completing the necessary intake forms provided to you by our intake coordinator, we will schedule you with one of our therapists to complete a comprehensive evaluation.

During the session the therapists will use formal evaluation measures to determine your child's individual profile to guide therapeutic sessions. A parent interview will be conducted as well to give our therapists a more in depth background of your child's interests, developmental milestones, and challenges in their natural environments.

From this a formal report will be documented and a plan of care will be created to address your child’s specific needs.

  • During your therapy evaluation:

    • Your therapist discusses frequency recommendations for therapy.

    • Your therapist asks you to provide weekly availability (days & times) to attend weekly recurring therapeutic appointments.

  • After your therapy evaluation:

    • We will thoughtfully match you with a therapist with expertise in the area for which you are seeking services.

    • We will reach out to you to schedule therapy times that align with your preferred availability provided during intake to add you to the recommended therapist's recurring schedule.

    • As you progress through with therapy, you and your therapist might decide to make changes to the planned schedule.

  • Attendance:

    • Our goal is to provide our patients with the best possible care. When you miss appointments, your child misses out on treatments and services that are important to your child’s development.

    • Your therapist will work with you to develop a plan that’s realistic for your family, to ensure attendance at your child’s therapy sessions.

    • If you exceed our company policy’s allowable missed appointments, or if you miss appointments without providing advance notice, it may result in discharge from therapy.
